SakeOf is a healthcare membership built around practical ways to navigate care: selected direct services for everyday needs, the freedom to generally choose any appropriately licensed provider, and advocacy to help make bills and fair pricing easier to understand. Start with the question in front of you—whether you need a quick clinical conversation, ongoing primary care, an in-person visit, or help sorting out a bill—and match it to the membership features you want active.
Use the calculator to explore membership options and potential costs, and the Savings Guide to understand the available services. Provider search can help you plan in-person care; Jordan can help you understand membership choices and what to check next.
When a non-emergency concern may be handled remotely, Zero Copay Telemedicine can be a convenient first step when selected and active. It offers access to a clinical conversation without starting with an office visit. If your concern needs an examination, test, or in-person follow-up, you can choose an appropriately licensed provider and arrange that care directly.
Rx and Prescription Benefits can support medication needs when selected and active. Check the current benefit terms for the medication and how to access it before relying on a particular price. Third-party benefits may have their own coordination restrictions, so confirm current vendor terms before combining arrangements.
Mental Health and Counseling may provide immediate problem assessment, supportive counseling, follow-up with the original counselor when available, crisis support, and referrals when more care is needed. Psychiatry, inpatient or emergency psychiatric care, neuropsychological testing, residential substance-use treatment, and medication costs are not automatically included merely because counseling is active.
Virtual Primary Care is a direct service with a $15 monthly adjustment when selected. The current bundled tier includes a dedicated physician relationship, ongoing virtual follow-up, an annual virtual wellness visit with health-risk assessment and stratification, and chronic-care or medication-management support when clinically appropriate for virtual care. Rx and Prescription Benefits, Zero Copay Telemedicine, and Mental Health and Counseling must remain active for this tier.

These Lima listings are useful starting points, not confirmation of an appointment or an individual price. Before planned care, ask the provider what the visit includes and what you may owe. For labs, imaging, specialist visits, hospital services, or follow-up, confirm the service and expected charges with the specific provider.
SakeOf members may generally use any appropriately licensed provider. For planned care, provider search can help you explore options, and SakeOf can help identify fair-price options. Contact SakeOf in advance for planned higher-cost care when reasonably possible; that creates an opportunity to discuss pricing and documentation before care takes place.
With Full Service, advocacy can help review bills and supporting documents, verify billing details, assess fair pricing, and provide negotiation support under program rules. An itemized bill is required. SakeOf may also request medical records, clinical notes, diagnosis or procedure details, provider documentation, billing records, or a signed medical-record release. Keep paperwork together and respond to requests promptly.
For larger eligible medical expenses, Full Service combines advocacy and fair-price review with voluntary member-to-member community funding. Sharing is discretionary; eligibility, documentation, medical necessity, fair-price review, participation status, program limits, exclusions, and available community funds all matter. No particular payment or sharing outcome is promised.
Complete documents should be submitted within six months of the service date. Submit balance bills, denials, or payment demands within 10 days of receiving them. Keep your membership current, maintain a valid payment method, and meet any applicable member commitment or feature-specific responsibility. Review the membership options to see how Full Service and Major Medical Only differ.
Depending on the membership features selected, options may also include maternity, dental and vision discounts, chiropractic or acupuncture, physical or occupational therapy, and durable medical equipment. Each has its own terms, so check the current feature details before scheduling or purchasing.
Maternity requires a continuous 12-month waiting period, and pregnancy must begin after that period is complete. Adding or restoring maternity after conception does not make that pregnancy eligible. Newborn medical needs after initial discharge are separate from the mother’s maternity event, and the newborn should be added to membership within 30 days of birth.
