A more guided way to approach healthcare in 44131

SakeOf is a healthcare membership designed to make care decisions feel more practical. Members can start with the kind of help they need—an everyday symptom, an ongoing primary-care relationship, a prescription, a local visit, or help understanding a bill—and then check which membership services and optional programs are selected and active for them.

The two primary membership options are Full Service and Major Medical Only. Full Service brings together member advocacy, fair-price review, and voluntary community sharing for eligible larger medical events. Direct-service and discount features may be available as options. Use the calculator to compare membership choices and the Savings Guide to understand the options; Jordan can help you get oriented.

Start with everyday care

Common symptoms

Zero Copay Telemedicine is designed for common, non-emergency illnesses and symptoms appropriate for virtual care. The current service partner describes access to board-certified physicians licensed in the member’s state, available 24/7, with no program limit on medically appropriate consultation frequency or duration. Clinical suitability, provider availability, technology, state law, and prescribing rules apply.

A clinician may recommend an in-person examination or other care. Labs, imaging, specialty care, procedures, and in-person follow-up are separate services; a recommendation does not itself arrange or include them. If a telemedicine clinician prescribes medication, its cost follows the active Rx program and its rules.

Ongoing care and counseling

Virtual Primary Care, when selected and active, offers a dedicated physician relationship, an annual wellness visit, a health-risk assessment, chronic-care follow-up, and medication-management support. It can be a starting point for ongoing primary-care conversations. In-person visits, labs, imaging, and specialist services remain separate care decisions.

Mental Health and Counseling can provide on-demand telephonic counseling, including assessment, supportive counseling, follow-up when available, and crisis support. When arranged and available, the feature may include up to three face-to-face counseling consultations per incident. Psychiatry, inpatient behavioral health, and medication costs are separate unless another active program expressly includes them.

Choose local in-person care when it fits

Sometimes a visit, test, or examination needs to happen in person. In Independence, Cleveland Clinic’s Independence Family Health Center lists primary and specialty care, Express Care, laboratory services, and an on-site pharmacy. Service hours vary, so check the location information or call before visiting. my.clevelandclinic.org

Cleveland Clinic also lists an Independence Express Clinic at 5001 Rockside Road as a walk-in location. Confirm current hours, services, and availability directly before going. my.clevelandclinic.org These listings give you local starting points, not a guarantee of an appointment or of what a particular service will cost.

You keep provider choice for in-person care. Before booking, ask the provider about the service, appointment availability, and expected charges, and check how the care relates to your active SakeOf options. A virtual recommendation or a local facility listing does not establish participation or a specific patient responsibility.

Check prescriptions and other optional features

If Rx and Prescription Benefits is selected and active, its current formulary includes approximately 70 commonly prescribed acute medications at $0 when program requirements are met and the medication is filled through the participating-pharmacy process. Use the current pharmacy tool to verify the medication and location before filling it. Chronic-maintenance, specialty, and other medications outside the active formulary are not automatically included at $0.

Other options may include maternity, dental and vision discounts, chiropractic or acupuncture services, and durable medical equipment. Their availability, eligibility, and program rules depend on the selected option. Review the Savings Guide and your membership details to see what is available to you.

For eligible physical or occupational therapy visits under the PT/OT feature, the member responsibility is $35 per visit and replaces the standard $500 event commitment for those visits. The feature has a shared 12-visit limit and a $780 annual maximum across enrolled membership. A physician order, referral, or plan of care may be required when clinically appropriate or required by law.

Get help with bills and larger eligible events

With Full Service, SakeOf advocacy can help members understand medical bills, verify billing details, review fair pricing, and provide negotiation support. For planned higher-cost care, contact SakeOf in advance when reasonably possible so you can understand the process and what records may be needed.

Keep an itemized bill for submissions. It should include service dates, provider information, and CPT or HCPCS codes when available. SakeOf may also request medical records, clinical notes, billing records, or a signed release to review eligibility, medical necessity, and fair pricing. Submit complete documents within six months of the service date; send balance bills, denials, or payment demands within 10 days of receiving them.

For a larger eligible medical event, the Full Service process includes the applicable member commitment, bill review, and voluntary community funding. The member commitment is a required member responsibility under the program, not a quote for a local service. Eligibility and community funding depend on program rules, documentation, and review. Keep membership current and respond to requests for information.
