It adds $18 per month, has a 12-month continuous active-selection waiting period before conception, and lists a $5,000 member responsibility and community-sharing limit of up to $25,000 per eligible pregnancy. The benefit period is pregnancy through six weeks postpartum for pregnancy-related care; eligibility and other program rules apply.
Dental and vision work differently: participating providers are paid directly at the applicable discounted rate, which varies and is not guaranteed at every provider. Routine dental and vision bills are not shared.
