This option has a monthly adjustment and requires Rx and Prescription Benefits, Zero Copay Telemedicine, and Mental Health and Counseling to remain active for the bundled tier.
Mental Health and Counseling provides unlimited on-demand telephonic counseling at no charge, with 24/7 access to master's-level professionals. Up to three face-to-face counseling consultations per incident may be arranged when available. The feature requires active Rx and Prescription Benefits and Zero Copay Telemedicine. Keep an itemized bill with service dates, provider information, and CPT or HCPCS codes when available. SakeOf may also request clinical or billing records to review a submission. Submit complete documents within six months of the service date, and submit a balance bill, denial, or payment demand within 10 days of receiving it. Keep membership current and meet the applicable member commitment under the program rules.
If a service is paid for or discounted through an optional feature, the same charge cannot be counted again as a separate sharing request unless SakeOf approves an eligible remaining balance. Keeping estimates, referrals, bills, and payment requests together makes it easier to provide the information needed for review.
With Full Service, a larger eligible medical event may be submitted for review and voluntary community funding under the program rules. Your applicable member commitment is the amount you are responsible for under your active membership terms; the amount and treatment of a particular event depend on those terms. Funding is voluntary. If you are considering the maternity feature, it is available with Full Service when offered and selected. It requires at least 12 continuous months of active membership with the feature selected before conception. The member responsibility is $5,000 per eligible pregnancy, with community sharing up to $25,000 per eligible pregnancy after that responsibility. The benefit period runs through six weeks postpartum for pregnancy-related care, subject to program rules.
SakeOf’s prescription benefits are designed primarily for medications commonly prescribed for acute conditions. A listed medication is available at no charge when it is filled through the participating-pharmacy process and prescribing and dispensing requirements are met. Check the current pharmacy tool before filling a prescription because the formulary and participating-pharmacy rules determine whether a specific medication is included.
A medication outside the current formulary may have a negotiated or discount price, but it is not automatically available at no charge or eligible for community sharing.
