The SakeOf approach: handle simple care simply, keep the freedom to choose where you go, know more about the price before you pay, and have real support when a medical need becomes expensive or complicated.

How members use SakeOf in real life

Local facts are one part of the decision. These stories show what it looks like after joining.

Start with the care you need

SakeOf is a healthcare membership designed to make paying for and navigating care more member-directed. Instead of beginning every decision with an insurance process, you can consider the kind of care you need, the available SakeOf options, and the provider that makes sense for the situation.

For a non-emergency concern, start by checking whether an active membership feature fits. For a hands-on evaluation, testing, imaging, specialist visit, or hospital service, look for an appropriate provider and ask about the service, appointment availability, and expected charges. The calculator and Savings Guide can help you explore membership options; Jordan can help answer questions as you compare them.

Use everyday care options when they fit

When the Zero Copay Telemedicine feature is active, it provides 24/7 phone or video access for medically appropriate, non-emergency urgent-care consultations at $0 per eligible consultation. Clinical appropriateness, state law, provider availability, technology, and prescribing rules apply. It is a low-friction place to ask about an urgent, non-emergency concern; use emergency services for emergencies.

If a clinician prescribes medication, check whether it is on the current acute-medication formulary and follow the participating-pharmacy process. The Rx and Prescription Benefits feature adds $10 per month and provides listed formulary medications at $0 when program requirements are met. The current formulary—not a general assumption about generic drugs—determines whether a medication qualifies. Confirm it in the current pharmacy tool before filling.

A prescription outside the formulary may have a negotiated or discount price, but it is not automatically $0 or eligible for community sharing. Chronic-maintenance, specialty, compounded, fertility, weight-loss, and mental-health medications require a separate active program that expressly provides them. Ask about the medication and pharmacy process before relying on a particular price.

Mental Health and Counseling and Virtual Primary Care are options to explore in membership materials. Their availability and terms depend on the options offered and selected; check the current details rather than assuming a service is active. Telemedicine is for appropriate non-emergency care, not emergencies.

Find in-person care around Ontario

Avita lists a hospital, outpatient medical offices, a walk-in clinic, a pharmacy, and The Jode Cancer Center at Ontario locations. These are useful starting points when you need local care, but a listing does not confirm that a particular service, clinician, appointment, or membership arrangement is available. Contact the facility directly to check current services and hours.

Avita lists its Ontario Walk-In Clinic and pharmacy at 600 Richland Mall, and medical offices at 600 Richland Mall and 2981 W. 4th St. The Jode Cancer Center is listed at 600 Richland Mall, Suite 206. Call ahead about appointments, current services, prescription availability, and charges; confirm participation with the provider and any applicable coverage source.

For care that involves a lab, imaging, specialist, hospital, or in-person follow-up, ask which services are being ordered and where they will be performed. Request expected charges from each provider involved. A clinic visit and related tests or follow-up can be separate expenses, so clarify what the quoted amount includes before scheduling when possible.

For countywide public-health services, Richland Public Health lists a non-emergency clinic and programs. Its service locations and availability vary; contact the department to ask what is offered, where, and what a specific service costs. Fees for many clinic services are described as sliding scale, but confirm the fee for your service directly.

Get help when a bill is hard to understand

Full Service includes advocacy, bill verification, fair-price review, and negotiation support. If a bill seems inflated or confusing, gather the itemized bill and ask what documentation is needed before submitting it. An itemized bill should show service dates, provider information, and CPT or HCPCS codes when available. Additional records or provider documentation may be requested for review.

Submit complete documents within six months of the service date. Submit a balance bill, denial, or payment demand within 10 days of receiving it. Avita lists patient financial services for billing questions; contact the provider about its bill and use the SakeOf process for questions about membership review. A review or negotiation does not guarantee a particular price or outcome.

Understand support for larger eligible events

For eligible larger medical events, Full Service combines the applicable member commitment with a process that may include voluntary community funding, subject to program rules and review. Check the Program Guide for the commitment and event-specific requirements before making decisions or assuming an expense is eligible. Share requested records and billing details promptly so the review can be based on complete information.

Symptoms can matter in pre-existing-condition review even without a formal diagnosis. The Program Guide describes review of an illness, injury, symptom, diagnosis, or treatment that existed or was known, or for which advice, diagnosis, or care was recommended or received, during the 24 months before membership began. Eligibility depends on documentation and clinical review. Ask Jordan or consult the guide about how a specific situation is evaluated.

Choose options that match your needs

If maternity support matters, it is an optional community-sharing feature with a $18 monthly adjustment when offered and selected. It requires continuous active selection for at least 12 months before conception. The member responsibility is $5,000 per eligible pregnancy, with sharing up to $25,000 after that responsibility, subject to the maternity rules. Review the full guide before relying on the feature.

Dental and vision discounts, chiropractic or acupuncture, PT/OT, and durable medical equipment are other options to review in current membership materials. Availability and terms depend on the active program. For DME, the guide says equipment expected to cost more than $300 requires advance review when reasonably possible; the annual community-sharing maximum is $1,000 across the enrolled membership. Check the guide before arranging equipment or treatment.

How do I ask about the cost of care at an Ontario provider?

Contact the provider directly and ask for the expected charges for the specific service, including any separate lab, imaging, or follow-up costs. Avita lists Ontario locations, but its listings do not establish an individual patient's price.
