The SakeOf approach: handle simple care simply, keep the freedom to choose where you go, know more about the price before you pay, and have real support when a medical need becomes expensive or complicated.

SakeOf is a healthcare membership designed to make care easier to organize and medical bills easier to understand. Instead of beginning every decision with an insurance-first process, members can start with the need in front of them: a common illness, a prescription question, ongoing primary care, an in-person visit, or a bill that needs review.
Your membership options shape the care path available to you. Check which features are selected and active, then use the SakeOf provider search, Savings Guide, or Jordan to identify a practical next step. For scheduled care, call the provider to confirm the service, appointment requirements, participation, and expected charges.
For common, non-emergency symptoms that are appropriate for a virtual visit, Zero Copay Telemedicine can offer a low-friction place to start when that feature is active. A clinician can assess the situation and may prescribe medication when clinically appropriate and legally permitted. Virtual care does not itself provide in-person follow-up, lab work, imaging, or specialty care that a clinician may recommend.
Rx and Prescription Benefits can support prescription needs when selected and active; check the current program details for how a particular medication is handled. Mental Health and Counseling is another optional service for members who have it active. Availability, clinical suitability, prescribing rules, and service-partner terms apply.
Virtual Primary Care is a direct service with $0 virtual primary-care access under the current service partner. It includes a dedicated physician relationship, ongoing virtual follow-up, an annual virtual wellness visit and health-risk assessment, and chronic-care and medication-management support when appropriate for virtual care. The current bundle requires Rx and Prescription Benefits, Zero Copay Telemedicine, and Mental Health and Counseling to remain active.
When you need a physical exam, testing, imaging, rehabilitation, or other in-person care, Madison has options to investigate. UH Madison Health Center lists urgent care, family and pediatric care, rehabilitation, and in-person and virtual visits. UH Madison Physician Offices lists primary care, same-day sick care, and an on-site clinical pharmacy. Call to check the service you need and appointment availability.
Cleveland Clinic’s Madison Medical Office Building lists Express Care, laboratory, imaging, family medicine, and other specialties. Confirm current hours and whether your service requires an appointment. Local service listings are a starting point, not confirmation that a specific clinician is available, participates in an arrangement, or will charge a particular amount.
For scheduled care, ask the provider for a service-specific estimate and confirm expected patient responsibility with the relevant provider and applicable coverage source. Labs, imaging, specialist visits, and in-person follow-up are separate services; a telemedicine recommendation does not make them part of the virtual visit. For life-threatening emergencies, call 911 or seek emergency care rather than using a routine virtual-care path.
Lake County also has public-health services through the Lake County General Health District, including immunization clinics, and a separate Lake County Free Clinic in Painesville. Clinic locations, appointment requirements, eligibility, services, and fees can vary; contact the organizations directly before making plans. The Free Clinic describes its care as no-cost, but confirm eligibility and current availability with the clinic.
If a bill seems confusing or unusually high, SakeOf’s Full Service advocacy can help verify billing details, review medical necessity and fair pricing, and support negotiation. For planned care, contact SakeOf in advance when reasonably possible. A review may require an itemized bill with service dates, provider information, and CPT or HCPCS codes when available; clinical or billing records may also be requested.
Submit complete documents within six months of the service date. If you receive a balance bill, denial, or payment demand, submit it within 10 days of receiving it. SakeOf reviews documentation and pricing under program rules; it may negotiate or reprice a bill, but the final handling depends on the review. An expense handled through an optional feature cannot be counted again as a separate sharing request for the same charge unless SakeOf approves an eligible remaining balance.
For a larger illness or injury, eligible care may include diagnostics, hospitalization, surgery, or medically necessary ambulance services when program requirements are met. SakeOf reviews eligibility, documentation, medical necessity, and fair pricing. When an event qualifies, voluntary community funding may help with eligible expenses according to the applicable program rules and member commitment.
Before planned treatment, contact SakeOf as early as reasonably possible and ask what documentation and steps apply. The member commitment and other program rules matter to how an eligible event is handled; review the current Program Guide for the details that apply to your membership. Do not assume that a provider listing, estimate, or recommendation alone establishes eligibility or a final amount.
Depending on your needs, membership options may include maternity, dental and vision discounts, chiropractic or acupuncture, physical or occupational therapy, and durable medical equipment. These options have their own selection, activation, eligibility, and program rules. Ask what is active in your membership before arranging care or relying on a discount or sharing feature.
For maternity, the program’s waiting-period and eligibility rules matter: pregnancy must begin after the continuous 12-month maternity waiting period is complete. Adding or restoring the feature after conception does not make that pregnancy eligible. Check the Program Guide and contact SakeOf before planning services.
