An itemized bill should include service dates, provider information, and CPT or HCPCS codes when available; SakeOf may request additional documentation to review eligibility, medical necessity, and fair pricing.
Submit complete documents within six months of the service date. If you receive a balance bill, denial, or payment demand, submit it within 10 days of receiving it. These steps allow the review to proceed with the information requested; they do not guarantee a particular outcome or price.
For larger eligible medical events, Full Service includes an applicable member commitment and may involve voluntary community funding after review under the program rules. Ask Jordan or consult your Program Guide to understand the commitment and process that apply to your membership and situation. Sharing is voluntary, and eligibility and review depend on the Program Guide and the documentation submitted.
Pregnancy has a separately described option: the maternity feature adds $18 per month and is available with Full Service when offered and selected. It requires continuous active membership and maternity selection for at least 12 months before conception. For an eligible pregnancy, the member responsibility is $5,000 and community sharing may be up to $25,000 after that amount, subject to program rules. Review the maternity terms before relying on this option; adding it after conception does not make that pregnancy eligible.
