Send balance bills, denials, or payment demands within 10 days of receiving them.
If an optional feature pays or discounts a charge, the same charge cannot also be counted as a separate sharing request unless SakeOf expressly approves an eligible remaining balance. Ask how to submit the expense rather than assuming how it will be treated.

It requires continuous active membership and maternity selection for at least 12 months before conception; the member responsibility is $5,000 per eligible pregnancy, with community sharing up to $25,000 after that responsibility, subject to program rules. Adding or restoring maternity after conception does not make that pregnancy eligible.
For durable medical equipment, the annual community-sharing maximum is $1,000 across the enrolled membership.
