Instead of paying a big monthly premium and still feeling on your own when care is needed, SakeOf combines practical everyday-care options, provider choice, advocacy, and community support into one membership experience.
The SakeOf approach: handle simple care simply, keep the freedom to choose where you go, know more about the price before you pay, and have real support when a medical need becomes expensive or complicated.

Estimate my householdSakeOf is a healthcare membership designed to make paying for and navigating care more member-centered. Instead of treating every need as the same kind of transaction, you can begin with the type of care you need, use available membership services when they fit, and choose a provider for local in-person care.
Your first step depends on the situation. A common, non-emergency illness may fit telemedicine; a need for an examination, test, or hands-on care may call for a local appointment. If you are unsure where to begin, Jordan or the provider search can help you explore next steps. For a life-threatening emergency, call 911.
When Zero Copay Telemedicine is active, members can access medically appropriate, non-emergency urgent-care consultations by phone or video at $0 per eligible consultation. The service is intended for common symptoms and illnesses that can be handled remotely; clinical appropriateness, provider availability, state law, technology, and prescribing rules still apply.
A telemedicine clinician may prescribe medication when appropriate and legally permitted. Rx and Prescription Benefits are a separate program: listed formulary medications can be available at $0 through the participating-pharmacy process when program requirements are met. Check the current pharmacy tool before filling a prescription. Non-formulary and maintenance medications may have a separate cost or require a different active program.
In-person follow-up, labs, imaging, procedures, and specialist visits are not included just because a telemedicine clinician recommends them. When those are needed, use the provider search and contact the provider to ask about appointment access, services, and expected charges.
If Mental Health and Counseling is selected and active, it includes on-demand telephonic counseling at no member cost, with face-to-face consultations available in specified circumstances through the program. It requires active Rx and Prescription Benefits and Zero Copay Telemedicine. Virtual Primary Care is another membership option to explore; check the current membership details for its availability and terms.
ACE Wellness Medical Center lists its address as 27102 Royalton Rd., Columbia Station, and describes physicals, exams and screenings, blood work, and other services. Call the center before scheduling to confirm current services, clinician qualifications, appointment availability, accepted payment arrangements, and charges.
For hospital care, UH Elyria Medical Center lists a 24-hour emergency room and multiple hospital services; its address is 630 East River St., Elyria, and its main phone is 440-329-7500. For emergencies, call 911. A facility listing is a useful starting point, not confirmation of availability, participation, or what an individual will owe.
Lorain County Public Health offers vaccination and other public-health services; clinic schedules, availability, and payment options can vary. The county agency also lists a Columbia Station library among local blood-pressure monitor resources. Call the agency to confirm current access and details. For nutrition support, Lorain County WIC’s Amherst/Lorain clinic offers nutrition education, breastfeeding support, and referrals; contact WIC about appointments and eligibility. Lorain County 2-1-1 can provide information and referrals, but confirm service availability and costs directly with each provider.
When a bill is confusing or seems unusually high, SakeOf’s Full Service advocacy can support bill verification, fair-price review, and negotiation. For planned higher-cost care, contact SakeOf in advance when reasonably possible. SakeOf reviews eligibility, medical necessity, documentation, and price fairness; an unreasonable price may need negotiation before an expense is eligible for consideration.
Keep itemized bills and requested records together. An itemized bill should include service dates, provider information, and CPT or HCPCS codes when available. SakeOf may also request clinical notes, medical records, diagnosis or procedure details, or a signed release. Submit complete documents within six months of the service date, and send balance bills, denials, or payment demands within 10 days of receiving them.
For larger eligible medical events, the program applies the relevant member commitment and may use voluntary community funding. Eligibility, documentation, medical necessity, fair pricing, and program rules matter; contact SakeOf before planned higher-cost care when reasonably possible. An expense paid or discounted through an optional feature cannot be counted again as a separate sharing request for the same charge unless SakeOf approves an eligible remaining balance.
Keep membership active, maintain a valid payment method, fund required asks, and submit requested information promptly. These steps help the program review an expense. They do not guarantee that a particular bill or service will be eligible for sharing.
Membership options may also include maternity, dental and vision discounts, chiropractic or acupuncture, physical or occupational therapy, and durable medical equipment. Check the current membership details to see what is available and whether an option is selected and active. Maternity has a 12-month continuous waiting period before pregnancy begins; adding or restoring maternity after conception does not make that pregnancy eligible. If you are considering maternity, review the current program rules early.
