Estimate my householdSakeOf is a healthcare membership designed to make care easier to plan, understand, and pay for. Rather than beginning with a search through coverage rules, start with the question in front of you: Do you need advice for a new concern, a prescription, ongoing primary care, an in-person examination, or help with a bill?
Your membership combines services, advocacy, fair-price review, and voluntary community sharing. You choose your care, and SakeOf can help you understand the process, review eligible bills, and identify the next step. The options and support available depend on your active membership features and the program rules.
For a concern that may be appropriate for a remote visit, Zero Copay Telemedicine is an optional feature. When selected and active, it provides access to telemedicine with a zero copay through the service partner. A clinician can help assess the immediate issue and discuss whether another kind of care is needed. It is not a route for emergencies.
Rx and Prescription Benefits is another optional feature. The program is designed primarily for medications commonly prescribed for acute conditions. Listed formulary medications may be available at zero dollars through the participating-pharmacy process when prescribing, dispensing, and other requirements are met. Check the current pharmacy tool before filling a prescription: formulary status, participating pharmacies, and vendor rules can change. Non-formulary or ongoing medications may have a separate cost and are not automatically included in the acute benefit.
If you want continuing support, Mental Health and Counseling may include problem assessment, supportive counseling, follow-up when available, and crisis support when that feature is active. Virtual Primary Care is a separate option with a dedicated physician relationship, ongoing virtual follow-up, an annual virtual wellness visit, and chronic-care follow-up or medication-management support when clinically appropriate. Its current bundled service tier requires the related prescription, telemedicine, and counseling features to remain active.
When you need an examination, a service that requires a facility, or care that is better handled in person, you can choose a local provider. In Bowling Green, the Wood County Community Health Center lists primary and preventive medical care, dental, behavioral health, and pharmacy services at 1840 E. Gypsy Lane Rd. It describes an income-based sliding fee scale and care regardless of income or insurance status. Call 419-354-9049 to ask about appointments, hours, eligibility, and expected charges.
Falcon Health Center, at 838 E. Wooster St., lists primary care and walk-in urgent care, along with pharmacy and other services. Its urgent care is for minor illnesses and injuries; wait times may vary, so call 419-372-2271 to confirm current hours and access. Falcon Health Pharmacy is located at the center; confirm medication availability and charges directly.
For a serious emergency, call 911. Wood County Hospital’s emergency department is at 950 W. Wooster St. If you need hospital care that is not an emergency, ask the provider about an estimate and any separate professional fees. Labs, imaging, specialist visits, in-person follow-up, and facility services can have their own charges; ask how each will be billed and whether it is eligible under your active SakeOf program.
If a bill seems confusing or high, Full Service includes advocacy and bill verification. SakeOf reviews documentation and may review or negotiate the price for fairness when applicable. That gives you a practical point of contact for questions about the bill, the provider’s charges, and what information is needed for review.
Keep an itemized bill with service dates, provider information, and CPT or HCPCS codes when available. SakeOf may request clinical notes, records, diagnosis or procedure details, other billing documents, or a signed release to review eligibility, medical necessity, and fair pricing. Submit complete documents within six months of the service date; submit a balance bill, denial, or payment demand within 10 days of receiving it.
For a larger medical event, the sequence is designed to clarify the bill and what happens next. SakeOf reviews eligibility and documentation, reviews or negotiates the price for fairness when applicable, and applies the relevant standard-event commitment or feature-specific member responsibility. Remaining eligible amounts may then be matched to voluntary community funds, subject to program limits and available funds. Approved amounts may be paid through your member wallet or card, or directly to the provider, depending on the process.
Before planned higher-cost care, contact SakeOf in advance when reasonably possible. Ask what documentation is needed, what member responsibility applies, and whether the planned service can be reviewed. Approval, eligibility, and available community funds depend on program rules; do not assume a provider estimate is the same as your final responsibility.
Households can consider optional features for specific needs. Maternity Care may be selected for eligible pregnancy-related services, subject to a continuous 12-month waiting period and the program’s limits and requirements. Pregnancy must begin after that waiting period; adding the feature after conception does not make that pregnancy eligible.
Other options include dental and vision discounts, chiropractic and acupuncture, physical and occupational therapy, and durable medical equipment. These features apply only when selected, active, eligible, and used under their program rules. Compare the feature details with the care you expect to use, and ask what charges remain separate.
