Provider availability, technology, state rules, clinical appropriateness, and prescribing rules apply. A clinician may recommend in-person follow-up, a lab, imaging, or a specialist; those are separate care decisions. A recommendation does not itself include the cost of those services.
Rx and Prescription Benefits are designed primarily for medications commonly prescribed for acute conditions. A medication on the current formulary may be available at $0 when the participating-pharmacy process and prescribing and dispensing requirements are met. Check the current pharmacy tool before filling a prescription, since the formulary and participating pharmacies may change.
A medication outside the formulary may have a negotiated or discount price, but that does not make it automatically $0 or eligible for community funding. Chronic-maintenance, specialty, compounded, fertility, weight-loss, mental-health, and other medications are included only when a separate active program expressly provides for them.
