If a medical bill seems unclear or unusually high, ask the provider for an itemized bill and an explanation of the charges. SakeOf may review eligible bills for documentation and fair pricing and may provide negotiation support through Full Service. An itemized bill should include service dates, provider information, and CPT or HCPCS codes when available.
Timing matters. Submit complete documents within six months of the service date. Submit balance bills, denials, or payment demands within 10 days of receiving them. SakeOf may also request medical records, clinical notes, diagnosis or procedure details, provider documentation, billing records, or a signed medical-record release to review eligibility, medical necessity, and fair pricing.

For a larger eligible medical event, SakeOf’s Full Service process can include bill verification, fair-price review, negotiation support, and voluntary community funding, subject to the Program Guide and review of the expense. A standard event commitment may apply. For eligible physical and occupational therapy visits under the PT/OT feature, the $35 feature-specific member responsibility replaces the standard $500 event commitment.
PT/OT services must be medically necessary and provided by an appropriately licensed therapist. The feature has a shared limit of 12 visits and a $780 annual maximum across the enrolled membership. A physician order, referral, or documented plan of care may be required when clinically appropriate or required by state law. Check whether the feature is active and whether the proposed service fits its terms before arranging care.
Eligibility review can consider an illness, injury, symptom, diagnosis, or treatment that existed, was known, or involved recommended or received medical advice or care during the 24 months before membership began—even if there was no formal diagnosis.
