SakeOf is a healthcare membership built around practical services, provider choice, advocacy, fair-price review, and voluntary community funding for eligible medical expenses. Instead of beginning with a complicated cost question, members can start with the care they need: see whether an everyday service fits, find an appropriately licensed provider when in-person care is needed, and ask for support when a bill or price is unclear.
Membership options let you select features that suit your household. The calculator and Savings Guide can help you compare those options; Jordan can help you explore a care path or prepare questions for a provider. For ZIP 93421, use provider search to begin researching care nearby, then verify the specific location, appointment availability, participation details, and expected price directly.
When Zero Copay Telemedicine is selected and active, it offers access to urgent-care telemedicine for common, non-emergency illnesses and symptoms appropriate for virtual care. The current service partner describes 24/7 access, unlimited medically appropriate consultations, and physicians licensed in the member’s state. A clinician determines whether a concern is suitable for a virtual visit. If you need emergency care, seek emergency services; a telemedicine visit does not itself include follow-up tests, imaging, specialist care, procedures, or an in-person visit.
If Rx and Prescription Benefits are selected and active, the service partner describes about 70 acute medications at $0 at participating pharmacy locations when the medication is included in the program. Check that the particular medication and pharmacy qualify before filling a prescription. Prescription costs follow the active Rx program, so a telemedicine prescription is not automatically cost-free.
Mental Health and Counseling, when selected and active, offers on-demand telephonic counseling, supportive follow-up, and crisis support. The service partner describes up to three face-to-face counseling consultations per incident when arranged and available. If you need more care, SakeOf can provide referrals to appropriate plans or community resources. Psychiatry, inpatient behavioral health, emergency psychiatric care, and medication costs are not included simply by activating counseling.
Virtual Primary Care can suit members who value an ongoing relationship rather than a one-time virtual visit. With the feature active and its required bundled services maintained, it includes a dedicated physician relationship, virtual follow-up, an annual wellness visit, a health-risk assessment, and chronic-care follow-up and medication-management support when appropriate for virtual care.
This option has a $15 monthly adjustment and requires active Rx and Prescription Benefits, Zero Copay Telemedicine, and Mental Health and Counseling for the current bundled tier. Compare the requirements and monthly adjustment in your membership options before choosing it. For needs that call for examination or treatment in person, use provider search or Jordan to identify possible providers and confirm details with the location.
Members may generally use any appropriately licensed provider. For a clinic visit, lab, imaging service, specialist, procedure, or hospital care, contact the provider to confirm that the location offers the service you need and has appointments available. Check participation details with both the provider and any plan that applies to you; a directory listing alone does not establish current availability.
For planned care, request a written estimate for the specific service. Ask whether it includes facility and clinician fees, and whether labs or imaging are billed separately. California’s HCAI publishes hospital cost-transparency resources, including chargemasters, which can provide reference information but may not represent an individual patient’s final amount. hcai.ca.gov
California’s DHCS Provider Information Network lets people search for doctors, dentists, hospitals, and clinics by ZIP code, county, or address, with filters for language and services. Use it to broaden your provider search, then contact the provider to check the service, current availability, and participation details that matter to you. www.dhcs.ca.gov
Full Service includes healthcare advocacy and bill review. SakeOf can review documentation, verify charges, assess fair pricing, and provide negotiation support. If you receive a bill that is confusing or seems inconsistent with what you expected, keep the itemized bill and related notices, then ask SakeOf how to submit them for review.
An itemized bill is required for submissions and should include service dates, provider information, and CPT or HCPCS codes when available. SakeOf may request medical records, clinical notes, provider documentation, billing records, or a signed release to review eligibility, medical necessity, and fair pricing. Complete documents should be submitted within six months of the service date. Send a balance bill, denial, or payment demand within 10 days of receiving it.
Arroyo Grande Community Hospital is a local hospital resource. For a planned hospital service, contact the hospital for a written, service-specific estimate and ask whether clinician fees are separate. The hospital determines its own financial-assistance eligibility and terms; contact it directly for current billing and assistance information.
For larger eligible medical events under Full Service, SakeOf provides advocacy and fair-price review, followed by the applicable member commitment and voluntary community-funding process. The standard event commitment is $500. Eligibility and documentation requirements depend on the Program Guide and review of the event, and community funding is voluntary. Use the calculator and Savings Guide to explore membership choices, and ask Jordan questions before making a decision.
Some selected features have different terms. For example, eligible medically necessary physical or occupational therapy under the PT/OT feature has a $35 feature-specific member responsibility instead of the standard event commitment. The feature has a shared limit of 12 visits and a $780 annual maximum across the enrolled membership. Services must be provided by an appropriately licensed therapist; an order, referral, or plan of care may be required when clinically appropriate or required by law.
Depending on the options selected and active, membership choices may also include maternity, dental or vision discounts, chiropractic or acupuncture options, or durable medical equipment benefits. Review the applicable feature details before scheduling care or making a purchase.
